Payment Options: Fees may be paid via cash, personal check, or credit card.

 

Insurance: Upon request, I will provide you with an invoice that you may submit to your insurance company for reimbursement. If you intend to seek reimbursement from your health insurance company, it is best to contact them prior to our first session to ask them the following questions:

• What is my deductible?

• What is my co-payment?

• How many sessions will be covered?

• What percentage of my bill will be reimbursed for services provided by an out-of-network provider?
